Continuity of Care Maturity Model
Going beyond EMRAM
Created to help optimize outcomes for health systems and patients alike, the Continuity of Care Maturity Model™ goes beyond Stage 7 of the Electronic Medical Record Adoption Model (EMRAM)sm. This global maturity model addresses the convergence of interoperability, information exchange, care coordination, patient engagement and analytics with the ultimate goal of holistic individual and population health management.
Similar to EMRAM, the Continuity of Care Maturity Model is comprised of eight stages (0-7), each requiring a unique set of criteria be met prior to advancing. “For years, the industry has asked ‘What comes after EMRAM Stage 7?’ This model is our answer,” said John Hoyt, HIMSS Analytics Executive Vice President. “After an organization has achieved EMRAM Stage 7, this model provides a roadmap to help them reach the industry’s overall goal of a true interconnected healthcare delivery model.”
The Continuity of Care Maturity Model was unveiled at HIMSS14 and the pilot is currently slated for launch in early 2015.
